You should find a ".trc" file related to this event in your USER_DUMP_DEST. This file should include the names of the objects involved as well as the offending SQL statements (I think -- it's been a while).

Hopefully, you can backtrack from the SQL statements to find the offending application modules. It's usually a design/implementation issue in the application...
