Ramon,

Can you tell us what error you get when using FORM, SQLPLUS? Are there any trace files? If, when using Forms you get ORA-1003, you may want to check Oracle Note: 74887.1, which details how to setup forms to work with IOT correctly. Also there are few bugs logged with IOTs when you have secondary indexes. If you have secondary indexes on the IOT, try dropping them and see if you get the same problems.

The actual error message and info from trace file (if available) would be helpful.
