1.- With profiles:

create profile bad limit
sessions_per_user 2

alter user taffarel profile bad;

2.-
select spid "OS PID" from v$process,v$session where paddr=addr and sid=&sid.;

And in other to solve your problem bounce the database and change in the init.ora the processes parameter. If you can't, start killing sessions inactive.
