ora-1041?

We just got an oracle error ora-1041 on an operation. The oracle error text is:

ora-1041: internal error. HOSTDEF extension does not exist.

In typical oracle clarity, the book says this:

Cause: The pointer to the HOSTDEF extension in HOSTDEF is null.

Can anyone give me a little more explanation on this?
