I am trying to create a new database OPS database/instance. While running the script using SVRMGRL get the following messages..

ORA-01501: CREATE DATABASE failed
ORA-00200: controlfile could not be created
ORA-00202: controlfile: '/dev/disk/dsk19a'
ORA-27038: skgfrcre: file exists

Any ideas, the sys ops say everything is OK.
