2nd Time listing- Performance manager in OEM on NT4.0 please help

 Hello guys,

Please help, I need to desperately connect to Diagnostic pack and performance manager, so please help in telling me relative to the following, as to what I should do and how to tackle the problem on hand.

I have tried most of every thing I know, and also looked around, ran out of options basically. so please help.
