I would read the upgrade instructions that you can get from Oracle's site. I have never gone wrong doing this. Of course, it should go without saying, take a full, cold backup of your database first.

Does the catalog (and catproc?) need to be run again? aren't there any 'incremental scripts' like it used to be available for migration from Oracle 7.2.3 to Oracle 7.3.4 etc..??

If this is on Unix,
I would do the following:

1. Take a good backup(cold and export). Preserve archives. Just in  case
2. Shutdown database and listener.
3. Blow the $ORACLE_HOME.
4. Install 8.1.6 by setting proper env etc.
5. Rerun the catalog scripts.
6. Restart the database
7. Check everything.

IMPORTANT: If ur system is Prod, test the strategy in a test env and evaluate the results first
