This means that your forms listener is not up or is waiting on different port address.
As I remember correctly (hopefully), port 9000 is for prod, 9001 for test and 9002 for vision.
11.0.3 puts everything properly, I've done many installation and absolutely no problems faced so far.
(There is small bug in 11.0.3 one hour install as far as report reviewer agent is concerened, but you face it only when you de-install one of the dbs.)
Check whether you have any erros in NT event log (check all three) Check that all services are up, firstly try to connect through the server itself(install client jdk/jinit here first) Also there are command scripts available in $ORACLE_BASE/admin/$SID/*.cmd , check which port address is configured for the db you are trying. Also after installation, you need to enable modules (not in vd11) using adaimgr and also preferably apply latest mini-packs (from metalink).

You also need to edit startup jini html file for internet explorer path related settings. Also make sure you are using IE5.

BTW check that you are trying to access the right DB, which you installed (prod/test/vd11).
