    Has your instance *ever* come up properly? If so, what has changed     since the last time it did? Did you add new instances? My bet is     that your semaphores are set way too low. To determine this, add the     number of prcoesses in your init.ora for all instances you have on     the box, and multiply it by 1.x where x is a number between 1 and 5,     the higher your system activity, the higher your x should be. When     you (or your system administrator) have changed this, reboot the box     and try again.

    Also, you may want to call Oracle and log a priority 1 TAR (down     production system)... what could it hurt?
