Error in lib_acc_claims.lib_update_gl_sl; Error occurred while inserting into acc_gl_table table : ORA-01502 index 'INS.X_ACC_GL_Table' or partition of such index is in unusable state ORA-0000: Normal

drop and recreate the index.
Did you do any sqlldr direct load lately?
