We have Oracle database 9.2.0.1 with 9.2.0.4 patch installed on it. Now we are installing 9.2.0.8 on it. Is cold backup is must before installing patch? What is the risk if we take hot backup. If we disconnect all the users and keeping only sys user and take hot backup will it be helpful. Kinldy advise.

Yes it is better to keep a cold backup. This is for safety. if things go bad then you may have to restore.anyway applying the patch requires oracle to be shutdown so better keep a cold backup.

And just to add, you should also consider doing a "cold backup" on the ORACLE_HOME (backup the software directory with oracle shutdown).
