what and all should i ask the person
from whom i am taking over.
please reply.. i am in an urgent need

It depends. If you are going to be incharge for that application, better to ask about how the application works and all the suggestions that you may need when you troubleshoot. If you are taking over the application from who designed and developed, you better take the data relation/integration/normalization charts. So that you would be in a position to analyze the relations between tables when you have to troubleshoot.
