If you have already installed ORACLE 8.0 Uninstall by
1. Press DEL key to remove the folder ORAWIN95 (Do not use SHIFT + DEL Key )
2. Go to Registry by REGEDIT Command
3. In Registry go to HKEY_LOCAL_USERS -> SOFTWARE -> ORACLE. Delete it
4. Reboot it

Fresh Installation
5. Now install ORACLE FORMS 4.5 OR 5.0 in say D:
that will create folder D:ORAWIN95
6. Reboot the machine
7. Now install ORACLE 8.0 in D: with home ORADATA
8. Create NET 8 Connection string say eg 'TEMP' using NET 8 Assistant
9. Copy the TNSNAMES.ORA to the folder where FORMS is installed
10. Now start the LISTENER by :
D:ORADATABIN > LSNRCT~1
(I dont remember exactly the .exe file for LISTENER)
LSNRCTL> START LISTENER
11. Now open FORMS 5.0 and in CONNECT Dialog box write the connection string that you created earlier using NET 8 CONNECTION S/W
