Similar problems were discussed previously; here are the search results for "excel" keyword in our Forms forum. One of these discussions, with attached code and a FMB file is here (How to export Data to excel?).

To successfully configure WebUtil, please open the Forms Help system click on the "Search" tab and search on "Config WebUtil" and review the following two articles:
Configuring WebUtil
Runtime Setup Checklist

Print the checklist and follow each step. If you still have problems, please post the output from the Java Console.
