Is it possible that we create a forms like matrix report. In here record orientation in both side.

Yes, but it is not an easy wizard-driven process. As you probably know, Forms offers form or tabular layout, so you'll have to do your own programming in order to achieve what you want. Have a look at this OTN Forms forum discussion; one of participants described what he did and how, so - maybe it'll help.
