What is block's layout style? Form or tabular? Could you attach form's screenshot which shows records you entered? Mark a record that was saved (or the ones that were not).

How exactly did you enter these two records? I guess you simply overtyped the first record's values. Here's what you should have done:
  • enter first record's values
  • create a new record (you'd do that by pressing the arrow DOWN key or by pushing the green PLUS button in Forms toolbar)
  • a new, empty record is displayed on the screen. If it was a tabular form layout, it would be more obvious; in a form layout, the first record you entered was (kind of) "moved up" and you don't see it any more
  • now, enter second record's values (the ones we see on the screenshot)
  • now, save changes (i.e. COMMIT

What is the result now?
